Rates for therapy services:

  • Initial Intake Appointment: $165

  • Therapy Session: $145

Insurance

Meadow Grove Counseling, LLC is in network with:

  • BCBS (not including Capital Blue Cross and Independence Blue Cross)

  • CHIP (Highmark)

  • Highmark

Behavioral health services may be covered in full or in part by your health insurance provider. Please contact your health insurance provider (the number is typically located on the back of your insurance card) to determine what services are covered by your plan and how much is compensated. 

The following questions may be helpful to ask your health insurance provider to gather details about your benefits:

  • Do I have behavioral health insurance benefits?

  • What is my deductible and has it been met?

  • What is the coverage amount per therapy session? What is my copay?

  • How many sessions per year does my health insurance plan cover?

  • Are teletherapy sessions covered?

  • Is written approval required from my primary care physician for services to be covered?

Out of Network

Receipts (superbills) can be provided to clients wishing to use their out-of-network benefits for health insurance providers Meadow Grove Counseling, LLC is currently not in network with. Some plans may reimburse some of your costs for out-of-network provider services and in those instances, you can submit the receipts to your provider for reimbursement. I would recommend reaching out to your health insurance provider prior to starting with therapy to determine how much they would reimburse you for behavioral health services, if a provider is not in network with them.

Payment

Fees are payable at the time of service. Acceptable forms of payment are cash, check, and major credit cards.

Cancellation Policy

If you miss or cancel a scheduled therapy appointment with less than 24 hours’ notice, you will be charged a fee of $60.

Good Faith Estimate Information

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
